Clubhouse of Suffolk is a recognized leader in state-of-the-art rehabilitation and support services that assist people whose lives have been disrupted by disabling forms of mental illness to lead vocationally-productive and socially satisfying roles and lifestyles.

Position: Psychiatrist
Compensation: To be discussed
Location: Riverhead and Ronkonkoma site immediately.
Hours: Eight (8) hours per week to start.
Start Date: ASAP

Position Description:
Within a New York State-licensed Personalized Recovery-Oriented Services (PROS) program: provide medication management, psychiatric assessments, health review and coordination of care with other medical providers, symptom management/education, and crisis assessment/intervention and coordination with a multidisciplinary team. Additional services may be required to support and meet EHR Meaningful Use standards. Services are provided under the supervision of the Medical Director.

Qualifications:
Commitment to working with people whose lives have been affected by psychiatric disabilities and co-occurring disorders. Candidates should be Board Eligible or Board Certified in Psychiatry. Candidates should also have a current DEA number, an active Medicare provider ID number, a Medicaid provider ID number, and a National Provider Identification (NPI) number. Strong computer skills/aptitude, as the agency uses a Certified Electronic Health Record (EHR) system for all charting, documentation, scheduling, and e-prescribing purposes. Candidate must be willing to participate in the agency's pursuit of NYS Medicaid EHR Incentives (details will be provided).

As the public health systems in New York State undergo massive changes, including the integration of behavioral and physical health care, our care management program is looking for impressive individuals with professional and personal experience in all areas of health-care to join our team.

Positions: Health Home Care Managers
Compensation: $35,000 plus highly competitive benefits package
Location: Riverhead, NY - travel throughout the Eastern Suffolk
Hours: 37.5 hours/week, some flexibility for eve/weekend
Start Date: Immediately

Position Description:
Care Managers establish supportive helping relationships with people who have significant behavioral and/or physical health conditions in order to assist them to achieve improved overall health and social functioning. Key activities include:
Coordinating and monitoring behavioral, physical health, and social services based on consumer-driven needs assessment and service plan;
Building and monitoring linkages with natural supports and formal community resources including hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.
Providing supportive, recovery- and wellness-directed coaching and counseling in-person and via telephone communication
Work is done primarily in the community and position entails travel throughout the East End. Agency vehicles can often be used but a safe, personal auto and a clean NYS Drivers License are required; mileage reimbursement provided. Care Managers work in teams and receive weekly supervision and as much support as needed. Great working environment; laptops and cell phones provided.

Qualifications:
A Bachelor's degree in a human service/health discipline and two years' experience is required. Candidates with Master's degrees related to physical/mental health are also encouraged to apply. Bi-lingual skills and personal recovery experience are highly encouraged to apply.

The successful candidate should possess or be quickly able to acquire the following skills and knowledge:
Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric and substance abuse disabilities and trauma as well as debilitating physical conditions;
Strong communicate with health professionals involved in the consumer's care;
Understanding of the key elements of recovery and wellness, including the role of family and self-help;
Sound organization skills including computer skills (agency uses electronic health records).
Basic knowledge of the public health/mental health system, public entitlement and social service programs;
Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.

Peer and Family Specialist Care Managers engage people who have significant behavioral and/or physical health conditions and family members in order to promote and achieve the individual's improved overall health and social functioning. Peer and Family Specialists work as key members of our care management teams, with emphasis on outreach and engagement and coaching roles. Key activities include:
Building and monitoring the consumer's use of linkages with natural supports and formal community resources including hospitals, outpatient clinics, rehabilitation, self-help, and social service providers.
Providing recovery- and wellness-directed coaching and counseling to individuals and families in-person and via telephone communication;
Provide consumer-driven needs assessment and service plans.
