Peer Wellness Coach (4 positions)
Clubhouse of Suffolk is a recognized leader in state-of-the-art rehabilitation and support services that assist people whose lives have been disrupted by disabling forms of mental illness to lead vocationally-productive and socially-satisfying roles and lifestyles. Through an award by Suffolk County Division of Community Mental Hygiene Services, we are expanding our range of services emanating from our current supportive case management program to include rehabilitation and support services to residents of adult homes.

Position Description:
We are seeking to hire and train four Peer Wellness Coaches to work with members of Clubhouse- Ronkonkoma and Synergy Center who are age 55+ to strengthen their linkages to primary health care, to engage in health improvement activities, and to reduce high-risk behaviors and health-risk factors. Peer Wellness Coaches will work as part of a new Geriatric Mental Health Project funded by the New York State Office of Mental Health. Coaches will report to the Clubhouse of Suffolk Director of Special Projects.
Peer Wellness Coaches will eventually be deployed to work as part- or full-time staff members within the Clubhouse-- Ronkonkoma of Suffolk and Synergy Center PROS programs.
Key Tasks and Responsibilities:

Empower peers through individual and group experiences to identify, choose, attain, and keep personal wellness and healthy lifestyle goals. |

To work with peers to increase their understanding of prevention, intervention, and harm-reduction with respect to specific medical and behavioral health conditions |

To collaborate with peers and their service providers to maximize the effectiveness of their recovery |
| • |
Assist peers to strengthen their readiness to actively pursue health wellness. |
| • |
Compile and share wellness and healthy lifestyle resources for peers and other staff or supporters. |
| • |
Selectively use self disclosure to inspire and support |
| • |
To effectively summarize and document services rendered |
Qualifications:
Applicants for the peer wellness coach position must:

Experience and commitment to model and promote personal responsibility for health and recovery. |
| • |
Posses and demonstrate sound communication skills including an effective ability to actively listen to and engage peers. |
| • |
Possess a valid and clean driver's license and your own transportation and a willingness to transport peers in your role |
| • |
Be available to commit to multiple days of overnight training in New Jersey |
