Position:                       Supportive Case Manager - Riverhead

 

Compensation:             Based on experience, plus an excellent benefits package (health & dental insurance, life insurance, pension plan, plus personal and vacation leave)

 

Location:                      Riverhead, NY

 

Hours:                          Full time @ 37.5 hours per week 8:30am – 4:30 pm (Monday-Friday)

 

Start Date:                    Immediately

 

Position Description:

Supportive Case Managers (SCM) establish helping relationships with a caseload of 28-30 people with psychiatric disabilities who have high service needs in order to planfully and aggressively assist them to connect to and utilize formal and natural resources critical to attaining and maintaining productive, healthy, and satisfying lifestyles.  Key activities include supportive counseling, rehabilitative goal planning, case advocacy, resource development and service linkage.  Some positions will work in tandem with an Intensive Case Manager while others will work independently.

 

Qualifications:

The successful candidate should possess or be quickly able to acquire the following skills and abilities:

Ø       Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric disabilities and trauma.

Ø       Sound communication and organization skills including computer literacy (word processing, internet communication and basic database management; Excel, Access, Word).

Ø       Basic knowledge of the public mental health system and recovery.

Ø       Proficiency in conversational Spanish a plus.

 

Education:

BA Required.  Candidates should possess a Bachelor’s degree in a Human Service discipline and/or two  years in Mental Health as well as a clean NYS DMV motor vehicle license and have a safe personal auto (except for 2 Eastern North and South Fork positions) to be used for carrying out the responsibilities of the position.

Position:                       Kitchen Unit Facilitator

                                    2 Positions, 1 in Ronkonkoma, 1 in Riverhead

Position Description:

A Clubhouse unit facilitator has a wide range of psychiatric rehabilitation responsibilities that emanate from the relationships formed in the Clubhouse work ordered day.  These responsibilities include, but are not limited to:

Ø       Advisement/supportive counseling with a caseload of 15-18 people including advocacy, crisis intervention and assistance with concrete needs.

Ø       Kitchen and dining room management including, team development, kitchen skill teaching, health oriented meal planning.

 

Qualifications:

The successful candidate should possess or be quickly able to acquire the following skills and abilities:

Ø       Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric disabilities and trauma.

Ø       Ability to facilitate multiple tasks in group based activities.

Ø       Strong communication skills including verbal and computer based.

Ø       Sound organizational skills.

Ø       Basic knowledge about Mental Health.

 

Education:

BA degree in a Human Service discipline is preferred; Associates within a Culinary education discipline considered.
